05794b46a7 GL_EXT_vulkan_glsl_relaxed - retarget gl_VertexID to gl_VertexIndex
instead of allowing for multiple declarations of the variable in the
resulting SPIR-V, instead use a retargeted mechanism to cause references
to gl_VertexID and gl_InstanceID to use the gl_VertexIndex and
gl_InstanceIndex symbol.

93b400f267 Fix issue for new unique id system. Add level bits to help verifying symbols and split symbol tables.
For intermediates rebuilding, now need manually amending level bits for redeclaring built-ins.

8. io mapping refine & qualifier member check & resolver expand (#2396)
* Code refine and adding missing features

1. Add new level for built in symbols.
2. Fix issues for structure members' qualifiers.
3. Global qualifier fix.
4. IO Mapper refine. Add support for checking with mangle names.

Current grammar would check block/struct member without its parent class's information.
So we split nesting level, and 'invariant' would only be checked within a struct.

* Format anonymous block names. Refine codes for symbols from all kinds of resouces.

* Fix writeonly check.

* Use LValueBase to find operator.

* Fix random null ptr issue.

* invariant check, stage in io mapping, reference parameter should be used and remove wrong codes introduced with ordering vector.

Use GLSLANG_ANGLE to strip features to what ANGLE requires
This change strips a few features similar to GLSLANG_WEB but doesn't
remove every detail like the latter.  It also hardcodes profile/version
to core/450.

In particular, TBuiltIns::initialize is specialized to remove most of
what is not supported or won't be supported by ANGLE.  The result of
this function is parsed with TParseContext::parseShaderStrings which is
a performance bottleneck.

This change shaves about 300KB off of ANGLE's binary size and reduces
the cost of SetupBuiltinSymbolTable to nearly a sixth.

deec1933e9 Web: Turn off includes, independent preprocessing path, fine tune all.
Saved about 21K, size down to 380K of MSVC x86 code.
Fixed one bug that needs to be looked at on the master branch:
The test for needing a Vulkan binding has a bug in it, "!layoutAttachment"
which does not mean "no layoutAttachment", because that is non-zero.
This is why some test and test results changed.

a4bfed129f WIP: track declared builtin type [proposal]
Marking as WIP since it might deserve discussion or at least explicit consideration.

During type sanitization, the TQualifier's TBuiltInVariable type is lost.  However,
sometimes it's needed downstream.  There were already two methods in use to track
it through to places it was needed: one in the TParameter, and one in a map in the
HlslParseContext used for structured buffers.

The latter was going to be insufficient when SB types with counters are passed to
user functions, and it's proving awkward to track the data to where it's needed.
This PR holds a proposal: track the original declared builtin type in the TType,
so it's trivially available where needed.

This lets the other two mechanisms be removed (and they are in this PR).  There's a
side benefit of not losing certain types of information before the reflection interface.

This PR is only that proposal, so it changes no test results.  If it's acceptable,
I'll use it for the last piece of SB counter functionality.

858c928ac7 Add basic HS/DS implementation.
This obsoletes WIP PR #704, which was built on the pre entry point wrapping master.  New version
here uses entry point wrapping.

This is a limited implementation of tessellation shaders.  In particular, the following are not functional,
and will be added as separate stages to reduce the size of each PR.

* patchconstantfunctions accepting per-control-point input values, such as
  const OutputPatch <hs_out_t, 3> cpv are not implemented.

* patchconstantfunctions whose signature requires an aggregate input type such as
  a structure containing builtin variables.  Code to synthesize such calls is not
  yet present.

These restrictions will be relaxed as soon as possible.  Simple cases can compile now: see for example
Test/hulsl.hull.1.tesc - e.g, writing to inner and outer tessellation factors.

PCF invocation is synthesized as an entry point epilogue protected behind a barrier and a test on
invocation ID == 0.  If there is an existing invocation ID variable it will be used, otherwise one is
added to the linkage.  The PCF and the shader EP interfaces are unioned and builtins appearing in
the PCF but not the EP are also added to the linkage and synthesized as shader inputs.
Parameter matching to (eventually arbitrary) PCF signatures is by builtin variable type.  Any user
variables in the PCF signature will result in an error.  Overloaded PCF functions will also result in
an error.

[domain()], [partitioning()], [outputtopology()], [outputcontrolpoints()], and [patchconstantfunction()]
attributes to the shader entry point are in place, with the exception of the Pow2 partitioning mode.

02467d8d94 HLSL: Wrap the entry-point; need to write 'in' args, and support 'inout' args.
This needs some render testing, but is destined to be part of master.

This also leads to a variety of other simplifications.
 - IO are global symbols, so only need one list of linkage nodes (deferred)
 - no longer need parse-context-wide 'inEntryPoint' state, entry-point is localized
 - several parts of splitting/flattening are now localized

26d3145334 HLSL default function parameters
This PR adds support for default function parameters in the following cases:

1. Simple constants, such as void fn(int x, float myparam = 3)
2. Expressions that can be const folded, such a ... myparam = sin(some_const)
3. Initializer lists that can be const folded, such as ... float2 myparam = {1,2}

New tests are added: hlsl.params.default.frag and hlsl.params.default.err.frag
(for testing error situations, such as ambiguity or non-const-foldable).

In order to avoid sampler method ambiguity, the hlsl better() lambda now
considers sampler matches.  Previously, all sampler types looked identical
since only the basic type of EbtSampler was considered.

75694fdacd Implement implicit conversions of function-call arguments (both in and out) as explicit conversions in the AST, through handleArgumentConversions().
Also
 - uniformly handle EvqConstReadOnly as an input argument in a function, with 
   isParamInput() and isParamOutput() queries in TQualifier.
 - provide a makeTemporary() in TQualifier, for erasing original qualification when making a temp
 - provide a makeInternalVariable() call to make a shader variable not seen in the shader source

a4351c55e8 More subtle checking for redeclarations:
- 300 doesn't allow built-in overload, while 100 does, while neither allows redefining
 - block name can't be reused for block name within the same interface 
   (though, the spec. isn't clear about that, it's easier than verifying member matches, will file bug on it)

f2ee3dd46a Add full support for uniforms with initializers, including intra-stage link validation of aggregate constant initializers.
This included 
 - encapsulating aggregate constants
 - removal of constant-aggregate comparison algorithms, instead using a flattened and direct std::vector comparison
 - adding structure type comparison for independently declared structures that still might match types

5f753e0222 Full thread safety working:
- don't use [] for map lookups, it can modify the map
 - copy up built-in symbols out of shared symbol table levels before modifying them
 - enforce shallow vs. deep TType copies
 - combine maxArraySize with the array dimensions vector, encapsulate
 - remove chaining of array types

38f3b890de Ensure the shared symbol table levels are read-only to make multi-threading safe. Also removed inadvertent extra copies of the symbol table shared across all stages.
