These introduce limited support for 8/16-bit types such that they can only be accessed in buffer memory and converted to/from 32-bit types.

- Use much simpler method to update implicit array sizes.
The previous overly complicated method was error prone.
- Rationalize all use of unsized arrays.
- Combine decorations when generating SPIR-V, to simplify
adding extensions.
There a couple functional problems, which when reduced down also led to
some good simplifications and rationalization. So, this commit:
- corrects "mixed" functionality: int[A] f[B] -> f[B][A]
- correct multi-identifier decls: int[A] f[B], g[C] -> f and g are independently sized.
- increases symmetry between different places in the code that do this
- makes fewer ways to do the same thing; several methods are just gone now
- makes more clear when something is copied or shared
This is part of the change to have desktop shaders respect precision
qualifiers on Vulkan, but since the defaults are all highp, and that's
different from ES fragment shaders, detect likely cases and warn about
them (but being careful to not be too noisy if it's unlikely to be a
problem).
- Support GL_AMD_shader_ballot (SPV_AMD_shader_ballot).
- Support GL_AMD_shader_trinary_minmax (SPV_AMD_shader_trinary_minmax).
- Support GL_AMD_shader_explicit_vertex_parameter
(SPV_AMD_shader_explicit_vertex_parameter).
- Support GL_AMD_gcn_shader (SPV_AMD_gcn_shader).
- Add new keyword int64_t/uint64_t/i64vec/u64vec.
- Support 64-bit integer literals (dec/hex/oct).
- Support built-in operators for 64-bit integer type.
- Add implicit and explicit type conversion for 64-bit integer type.
- Add new built-in functions defined in this extension.
This fixes a bug where a token that could be a keyword in one version
is not a keyword in another version, but treated like a non-member after
a "." dereference.
There will be subsequent commits to refine semantics, esp. version-specific semantics,
as well as I/O functionality and restrictions.
Note: I'm getting white-space differences in the preprocessor test results,
which I'm not checking in. I think they need to be tagged as binary or something.
New extensions in glext.h follow the pattern:

Versions.h tries to declare:
const char* const GL_ARB_texture_rectangle = "GL_ARB_texture_rectangle";
Which means, if you've included glext.h before Versions.h, that the
compiler will see "const char* const 1 = ...", and rightly refuse to
continue.
The ham-fisted approach taken here is to rename the variables in
Versions.h with a leading underscore. This does sort of undermine the
comment about "better to have the compiler do spelling checks", but.
