iOS 8 does not support `thread_local`, which is still in use.
Another approach will have to be found.
This change is a revert of the following changes:

glslang is using C++ 11, which has first class support for variables of the `thread_local` storage class.
By dropping the use of the `OS_[GS]etTLSValue`, we can simplify the logic, and have it support a thread-local default allocator if none is provided.

Addresses step 4 of #976, overlaps #916.
For each pool, now, it is newed, remembered, and freed by the same entity,
rather than having a mix (thread finalize freeing current pool) that could
lead to double freeing of the same pool. It is quite rational and simple now.
This will enable reinstalling process and thread tear down.

- replace #include comment about no location
This fixes various issues related to gcc's strict-aliasing warning
by using unions. It also handles various cases hit with
gcc's missing-declarations warning.
* Linux folder has been renamed to Unix, to match defines and so that it
compiles on OS X.
* This removes the need for a per-platform include search path for the
right OS folder
- the new C++ style interface now stands on its own, with the addition of glslang::InitializeProcess() and glslang::FinalizeProcess()
- more "global" pool names from a decade ago are fixed to be thread names
- StandAlone.cpp fully uses one of the old-style interface or new C++ style interface

- doing prescan of shader to know version/profile before parsing it
- putting precision qualifiers on built-in ES symbols
- getting most built-in state correct for core/compatibility/missing profile
- adding gl_VertexID and gl_InstanceID, among other ES 300 built-in symbols
- adding the ES 300 gl_Max/Min constants
- accepting shaders that contain nothing but whitespace without generating an error
